The Bottom Line: I recommend it to those who are obese since it helps you to achieve fast results, but you must be prepared to change your eating habits.

I have nothing but good things to say about Optifast. I think of myself as a picky eater and I don't mind the taste of the shakes at all. I find that the vanilla tastes like soft serve ice cream and chocolate is good too.
You can also use instant hazelnut flavoured coffee or artificially sweetened flavoured water (lemon, orange, strawberry and raspberry) for variety. Also, sometimes I add a little vanilla extract or another flavour. When at home, I use the blender to make an icy cold milkshake!
The problem with other diets for me is feeling like I will never see the light at the end of the tunnel. I am a woman, 5'2" and I was 210 lbs when I started. I felt overwhelmed by the necessary weight loss and often gave up on other programs. The first time I did the Optifast program I lost almost 30 lbs in 10 weeks, mind you I did not exercise. A year later I am 180 lbs after gaining back 20 lbs thanks to binge eating. I lost it gradually (October to April) by eating well and attempting to control the binges.
I have just started the shakes again for a much needed boost to my weight loss efforts. I'm not sure how long I will stay on the program (it's only day 4), but I would like to stay on the program 6 to 12 weeks to lose 20 to 30 lbs. It will depend on how long the doctor will allow me to continue.
This time around I am much more active thanks to a pedometer! It's really motivating as it helps me to realize when I have been too lazy.
Like some others have mentioned, the weight loss is very motivating, but you have to use this time to think about why you are an overeater and develop strategies for not overeating once you return to food. My problem is not that I don't understand what is and isn't healthy; I am a binge/emotional eater with an all or nothing attitude.
My program included a behavioural therapist for group sessions; however, I did not find it that beneficial. I would prefer one-on-one. I have had lots of time to think about my issues and I am trying to conquer my destructive behaviour (e.g., all or nothing attitude). It will be a life battle, I'm sure.
Anyway, if you have a substantial amount of weight to lose, I fully recommend this.
